counter not incrementing for RAM with built-in counter

I am really sorry for not posting original question as whenever I try to add tags the tab refreshes and all the typed data is encrypted. 

Kindly help me as its quite simple problem but somehow I dont understand why I am I having it.

looks like you are not driving state but next state so add a statement to drive it on the clk edge.
